Making Your Appointment

Please call our office at (225) 767-1151 to schedule an appointment at your convenience.


Please try to notify our office within twenty-four (24) hours if you are unable to keep your appointment.


Please call (225) 767-1151 for all emergencies. One of our physicians is available 24 hours a day, seven days a week for your medical needs.

Patient Registration

If you are a new patient, you can complete the new Patient Forms on line at our website at  Click on “Patient Forms” on the left hand side.

Telephone Calls

The physician’s nurse will initially handle all calls. The patient’s chart will have to be reviewed prior to the physician or nurse returning your call so that your problem can be handled properly.  Please tell the nurse if you feel your problem is an emergency.  Please leave a number where you might be reached because our routine calls are returned within 24 hours if this is not an emergency. 

Prescription Refills

All prescription refill requests should be called during regular office hours so that your medical records are available for the physician to review.  Routine prescription refills will not be given after our normal office hours or on weekends.


Our physicians practice at Our Lady of the Lake Regional Medical Center, Baton Rouge General Medical Center, Mid City and Bluebonnet location, Greater Baton Rouge Surgical Center,  St. Elizabeth Hospital and Lane Memorial Hospital.
